Create step-by-step online forms

Forms are the lifeblood of many online sites.. registration, payment, calendars etc. and if they can be presented as simple steps the chances of them being filled in correctly are higher.
Here is a great implementation of creating a step-by-step form.  It won't let you progress to the next stage unless you've completed the previous one correctly.  As a bonus, it's styled really nicely as well!
